To help ensure a smooth check-in process, please bring your dental insurance card and arrive a few minutes early if you have not completed your new patient forms in advance. It’s also helpful to bring a current list of any medications you are taking. If your physician has recommended antibiotics prior to dental treatment, please follow those instructions. If you are unsure whether this applies to you, we recommend contacting your doctor before your appointment.
